510(k) vs. PMA: Cost, Timeline, and Evidence Requirements Compared
What the 510(k) and PMA routes really cost in user fees, review time, evidence and post-market obligations — and how to build the difference into a funding plan.

The gap between a 510(k) and a PMA is not a matter of degree. It is two different businesses. One is a comparison exercise you can often complete with bench and validation data inside a year. The other is a clinical development programme with a manufacturing inspection attached, measured in years and millions.
Teams routinely underestimate the second and over-promise on the first. Here is the comparison in the terms that matter to a board: money, calendar, evidence and what happens after clearance.
User fees
FDA charges a standard fee for each submission type, set annually under the Medical Device User Fee Amendments and published before each fiscal year begins. Two things are worth knowing before you budget.
First, the order of magnitude is not close. A PMA application fee runs into the hundreds of thousands of dollars; a 510(k) fee is a small fraction of that. Second, small business status matters enormously. Firms qualifying as a small business — broadly, with gross receipts below the published threshold — pay a substantially reduced fee, and for the first PMA a qualifying small business may have the application fee waived entirely. Apply for small business certification early; it is an annual process and it cannot be applied retroactively to a submission already filed.
Always check the current fiscal year figures on FDA's MDUFA page rather than a number you saw in a deck. They move every October.
The submission fee is also the smallest line in the budget. Testing, clinical work, consultants and quality-system build dominate both routes.
Review clock versus elapsed time
FDA reports decisions against MDUFA performance goals, and those goals are stated in FDA days — the time the file sits with the agency, excluding the clock stops while you answer an additional information request.
For a traditional 510(k), the goal is on the order of 90 FDA days. Real elapsed time for most sponsors is longer, because nearly all submissions draw at least one additional information request and the response window can run to 180 days.
For a PMA, the goal is on the order of 180 FDA days for an original application, but elapsed time routinely runs well beyond a year once amendments, advisory panel scheduling where applicable, and the preapproval facility inspection are counted.
Plan on the elapsed figure, not the goal. The difference between the two numbers is where investor confidence is usually lost.
Evidence
This is the real divide.
A 510(k) is built on substantial equivalence. You identify a legally marketed predicate, compare intended use and technological characteristics, and demonstrate through performance data — bench testing, biocompatibility, electrical safety, software verification and validation, sometimes limited clinical data — that any differences do not raise new questions of safety and effectiveness. Guidance documents and recognised consensus standards tell you what testing FDA expects for your device type. Clinical data is the exception, not the rule.
A PMA is built on valid scientific evidence. You are demonstrating reasonable assurance of safety and effectiveness on its own terms, which in practice means a pivotal clinical study, usually run under an investigational device exemption, plus complete manufacturing information and a successful preapproval inspection of your facility and quality system. There is no predicate to lean on and no shortcut through comparison.

Side by side
Question answered: 510(k) asks whether the device is substantially equivalent to a predicate. PMA asks whether there is reasonable assurance of safety and effectiveness.
Typical device class: 510(k) covers most Class II. PMA covers Class III.
Clinical data: usually not required for 510(k); almost always required for PMA.
Manufacturing inspection before decision: not routine for 510(k); expected for PMA.
FDA decision goal: around 90 FDA days for 510(k); around 180 FDA days for an original PMA.
Outcome: 510(k) receives a clearance letter. PMA receives an approval order.
Changes after market: 510(k) changes are assessed against FDA's change guidance, and significant changes require a new 510(k). PMA changes go through supplements, of several types, each with its own fee and review clock.
What happens after the decision
Post-market obligations are the line item teams forget.
After a 510(k), you live inside FDA's guidance on deciding when a change to an existing device requires a new submission. Every meaningful design, materials, labelling or software change needs a documented assessment, and the file has to survive an inspector reading it two years later.
After a PMA, you file periodic post-approval reports, you may carry post-approval study commitments, and any change to the device, labelling, manufacturing process or facility runs through a PMA supplement. Budget for supplements as a recurring cost, not a one-off.

Building it into the plan
Three practical steps.
Apply for small business certification before you file anything, and confirm the current fiscal year fee schedule rather than a remembered figure.
Budget the elapsed timeline, with one additional information cycle assumed, and hold a contingency for a second.
Model the reimbursement path in parallel. A PMA-grade evidence package often satisfies payer questions that a 510(k) package never touches — which means a cheaper regulatory route can still be the slower commercial one.
